Background:

  • Hepatitis B virus (HBV) infection is a global health concern.
  • HBV-associated membranous nephropathy (MN) is a recognized complication.
  • The specific antigens involved in HBV-MN pathogenesis require further elucidation.

Observation:

  • An 18-year-old Korean male presented with a 9-year history of renal disease.
  • The patient tested positive for hepatitis B surface antigen (HBsAg), hepatitis B e antigen (HBeAg), and antibody to hepatitis B core antigen (anti-HBc).
  • Immunofluorescence revealed glomerular deposits of HBeAg, IgG, C3, and Clq, but not HBsAg or HBcAg.

Findings:

  • Glomerular HBeAg staining was detected using FITC-monoclonal anti-HBe F(ab')2 fragments.
  • This is the first reported case of glomerular HBeAg staining in a non-Japanese patient.
  • The co-occurrence of HBeAg in serum and glomerular deposits supports its causal role in HBV-associated MN.

Implications:

  • HBeAg may play a direct role in the development of membranous nephropathy in HBV-infected individuals.
  • This finding expands the understanding of HBV-MN pathogenesis beyond Japanese populations.
  • Further research is warranted to explore HBeAg as a therapeutic target in HBV-associated kidney disease.
