Summary

Conservative surgery for complex trigeminal schwannomas can be valuable. This approach led to spontaneous tumor shrinkage, highlighting challenges in assessing treatment responses for these benign intracranial tumors.

Background:

  • Trigeminal schwannomas are rare intracranial tumors, accounting for a small percentage of all brain tumors.
  • Effective management necessitates understanding tumor behavior and treatment outcomes.

Observation:

  • A 36-year-old male presented with symptoms including headaches, dizziness, and cognitive decline.
  • MRI revealed a large lesion in the Meckel's cave extending into the middle and posterior fossae, causing obstructive hydrocephalus.

Findings:

  • Subtotal resection of the posterior fossa component was performed to alleviate brainstem compression.
  • Pathology confirmed trigeminal schwannoma; residual tumor demonstrated spontaneous volumetric shrinkage post-surgery.

Implications:

  • Planned conservative surgery is a viable strategy for complex trigeminal schwannomas.
  • Interpreting treatment responses for benign schwannomas, whether surgical or radiation-based, presents unique challenges.
