Topical steroid therapy is more effective for treating childhood phimosis than placebo or manual reduction. This systematic review provides evidence for clinical options before circumcision.
Area of Science:
- Pediatric Urology
- Dermatology
- Evidence-Based Medicine
Background:
- Phimosis is a common condition in pediatric patients.
- Treatment options for phimosis include topical steroids, manual reduction, and circumcision.
- The efficacy of topical steroids requires systematic evaluation.
Purpose of the Study:
- To systematically review the therapeutic efficacy of topical steroids in children with phimosis.
- To provide data supporting clinical decisions for pediatric phimosis management.
- To compare topical steroid therapy with placebo and manual reduction.
Main Methods:
- Systematic review of original studies on topical steroid therapy for pediatric phimosis.
- Searched literature up to August 2014.
- Meta-analysis performed using REVMAN 5.2 software with quality assessment via Cochrane handbook.
Main Results:
- Eleven studies with 1669 patients were included.
- Topical steroids showed significantly higher efficacy compared to placebo or manual reduction (OR 7.46, p < 0.00001).
- Subgroup analyses confirmed significant efficacy against both placebo (OR 5.04, p < 0.00001) and manual reduction (OR 16.28, p < 0.00001).
Conclusions:
- Topical steroid therapy is a more effective treatment for pediatric phimosis than placebo or manual reduction.
- Topical steroids can be considered as a treatment option prior to circumcision.
- Further research may be needed to determine optimal steroid types and dosages.
