Background:

  • Many advanced cancer patients admitted to the intensive care unit (ICU) do not experience prolonged survival.
  • There is a lack of established criteria for appropriate ICU admissions in advanced cancer cases.

Purpose of the Study:

  • To establish and evaluate criteria for reasonable ICU admissions in patients with advanced, incurable cancer.
  • To determine the potential impact of these criteria on reducing inappropriate ICU admissions.

Main Methods:

  • Developed four criteria for reasonable ICU admission: post-procedure complication, recent cancer diagnosis, ECOG performance status 0-1, and life expectancy >6 months.
  • Reviewed charts of patients with advanced, incurable cancer who died in the ICU between 10/2005 and 10/2010.

Main Results:

  • Out of 421 ICU deaths, 52 had advanced, incurable cancer.
  • 47% of patients not meeting any proposed criteria had potentially inappropriate decisions (PIDs).
  • Applying the criteria suggested a potential reduction of 37% in inappropriate ICU admissions.

Conclusions:

  • Incorporating proposed criteria into ICU admission guidelines could significantly decrease inappropriate admissions for advanced cancer patients.
  • Increasing potentially inappropriate decisions alone is unlikely to substantially reduce inappropriate ICU admissions.
