Summary

Critically ill cancer patients in the ICU can experience ischemic stroke (IS) with unique features, often linked to cancer itself. Hemiparesis is a key symptom suggesting IS in these patients.

Background:

  • Cerebrovascular diseases frequently cause neurological symptoms in cancer patients.
  • Ischemic stroke (IS) in cancer patients is studied, but data on critically ill patients in intensive care units (ICUs) is limited.

Purpose of the Study:

  • To investigate the clinical characteristics and diffusion-weighted imaging (DWI) lesion patterns of ischemic stroke (IS) in critically ill cancer patients admitted to the ICU.
  • To compare these features between patients diagnosed with IS and those without.

Main Methods:

  • Retrospective evaluation of patients undergoing brain MRI for suspected IS in the ICU.
  • Comparison of clinical characteristics and DWI lesion patterns between IS and non-IS groups.

Main Results:

  • 49% of patients were diagnosed with IS; multiple DWI lesions were common.
  • Thrombotic events were more frequent in the IS group.
  • Hemiparesis at suspicion was a significant predictor of IS (adjusted OR 5.339).

Conclusions:

  • Ischemic stroke in critically ill cancer patients during ICU stays exhibits distinct characteristics.
  • These features may be associated with cancer-related mechanisms.
