Assessment of the Abdomen I: Inspection and Auscultation

3.0K
Introduction
The abdominal examination is a cornerstone of clinical medicine, serving as a critical tool in diagnosing various gastrointestinal (GI) diseases. It involves a systematic approach that includes inspection and auscultation, each with distinct yet complementary roles in assessing the abdomen. This article will delve into these two primary methods healthcare professionals use to examine the abdomen.

Assessment of the Rectum and Anus

1.8K
Evaluating the rectum and anus plays a crucial role in conducting a thorough physical examination of the gastrointestinal system. Although it may be uncomfortable and often embarrassing for the patient, it holds immense diagnostic value, particularly in detecting gastrointestinal diseases and abnormalities. This guide will explain how to perform this assessment using inspection and palpation methods.

Endoscopic Retrograde Cholangiopancreatography (ERCP) is a diagnostic procedure that combines endoscopy and fluoroscopy to diagnose and treat conditions related to the bile ducts, pancreatic ducts, and gallbladder. This procedure is beneficial for identifying and addressing blockages, gallstones, strictures, and tumors within the biliary or pancreatic systems. ERCP is both diagnostic and therapeutic, offering the ability to visualize and treat identified problems in one session.

Assessment of the Abdomen II: Percussion

3.2K
Percussion is a fundamental technique used to assess the liver, spleen, and abdominal organs by tapping the abdomen and interpreting the resulting sounds. This method helps identify fluid, distention, and masses through variations in sound, such as the high-pitched tympany of air-filled areas and the dullness of solid masses. Understanding how to percuss these organs provides valuable information for healthcare professionals in diagnosing conditions early.

Methods of Documentation V: CBE

1.5K
Charting by Exception, or CBE, is a method of documentation used in healthcare, particularly in nursing, that focuses on documenting only significant or abnormal findings rather than recording every detail. This approach aims to streamline the documentation process, improve efficiency, and ensure that healthcare providers can quickly identify deviations from normalcy in patient assessments.
In CBE, healthcare professionals establish predefined standards of practice that define what constitutes...

Summary

This audit of electroconvulsive therapy (ECT) in Iraq found resource gaps and noted its continued use for schizophrenia. Further training and exploration are recommended for modified ECT practices.

Area of Science:

  • Neuroscience
  • Psychiatry
  • Healthcare Management

Background:

  • Electroconvulsive therapy (ECT) is a vital treatment for severe mental health conditions.
  • Understanding current ECT practices in Iraq is essential for quality improvement.

Purpose of the Study:

  • To audit electroconvulsive therapy (ECT) practices in Iraq.
  • To assess adherence to local standards and identify resource limitations.

Main Methods:

  • A national audit was conducted across 10 Iraqi hospitals providing ECT.
  • Data were collected via questionnaires and patient case note review (first 6 months of 2013).

Main Results:

  • Resource shortcomings were identified, including inadequate clinic rooms and lack of EEG monitoring.

  • 251 patients received ECT; bilateral ECT and general anesthesia were common.
  • Schizophrenia was the primary indication, followed by depression and mania.
  • Conclusions:

    • Modified ECT practices require further development to ensure patient safety and efficacy.
    • The widespread use of ECT for schizophrenia warrants further investigation and targeted training.
