Purpose of the Study:

  • To identify predictive factors for patient and graft survival in patients undergoing repeat liver retransplantation (RRT).

Main Methods:

  • Retrospective analysis of 2940 liver transplantations in 2477 patients from 1985-2012.
  • Inclusion of patients undergoing third, fourth, and fifth liver transplantations.

Main Results:

  • Pre-operative vasopressor support, extra-hepatic sepsis, primary non-function (PNF), MELD score, urgent RRT, creatinine, and prothrombin ratio were associated with 90-day survival.
  • Systemic sepsis (OR=12.8) and vasopressor support (OR=4.7) predicted post-operative mortality.
  • Systemic sepsis (OR=6.4) and RRT between 1985-1999 (OR=3.6) predicted long-term mortality.

Conclusions:

  • Repeat liver retransplantation (RRT) is a valid option for carefully selected patients.
  • Selection criteria should prioritize patients without extra-hepatic sepsis and vasoactive drug support.
  • Fourth and fifth RRTs may be considered on a case-by-case basis despite high mortality risks.
