Summary

This study found no significant link between cerebral blood flow (CBF) and consciousness levels in patients with subarachnoid hemorrhage (SAH). Further research is needed to understand altered consciousness in SAH patients.

Background:

  • Altered consciousness in high-grade aneurysmal subarachnoid hemorrhage (SAH) etiology is unclear.
  • Hypothesis: Decreased cerebral blood flow (CBF) in consciousness-critical brain regions contributes to altered consciousness.

Purpose of the Study:

  • To investigate the relationship between CBF and consciousness in aneurysmal SAH patients.
  • To analyze CBF in default mode network (DMN) nodes and correlate it with clinical severity scores.

Main Methods:

  • Retrospective analysis of arterial-spin labeled (ASL) perfusion MRI in 12 aneurysmal SAH patients.
  • CBF measurement in DMN nodes (medial prefrontal cortex, thalami, posterior cingulate cortex).
  • Correlation analysis with admission Glasgow Coma Scale (GCS), Hunt and Hess (HH) class, and MRI GCS.

Main Results:

  • No significant correlations were found between nodal CBF and admission GCS, admission HH class, or MRI GCS (p > 0.05).
  • High inter-rater reliability for nodal CBF measurements (r = 0.71, p = 0.01).

Conclusions:

  • This pilot study did not establish a significant correlation between CBF in DMN nodes and consciousness levels in SAH patients.
  • Limitations include small sample size and variable ASL acquisition timing.
  • Larger prospective studies are required to confirm the role of cerebral perfusion in SAH-related consciousness alterations.
