Background:

Purpose of the Study:

  • To compare perfusion characteristics in different regions of solid thyroid nodules using CEUS.
  • To investigate the relationship between CEUS perfusion parameters and MVD in thyroid nodules.
  • To assess the diagnostic value of perfusion parameters in differentiating benign from malignant thyroid nodules.

Main Methods:

  • CEUS was performed on 59 patients with solid thyroid nodules.
  • Time-Intensity Curve (TIC) analysis was used to obtain perfusion parameters: Peak Intensity (PI), Time to Peak (TP), Area Under the Curve (AUC), and Mean Transit Time (MTT).
  • Immunohistochemical staining assessed MVD in surgical specimens; Spearman correlation analyzed quantitative parameters and MVD.

Main Results:

  • Malignant thyroid nodules (papillary carcinomas) showed statistically significant differences in PI, AUC, and MVD between normal tissue, tumor edge, and tumor center.
  • Benign thyroid nodules (nodular goiters) did not exhibit significant differences in these quantitative parameters or MVD across regions.
  • PI values for both carcinomas and goiters positively correlated with MVD (r=0.819, r=0.838, P<0.05).

Conclusions:

  • Variations in CEUS perfusion parameters provide a valuable basis for differentiating benign and malignant thyroid nodules.
  • Perfusion parameters are associated with MVD, reflecting microvessel distribution in neoplasms.
  • CEUS-derived perfusion metrics may aid in evaluating tumor angiogenesis.
