Background:

  • Portfolios are workplace tools for coaching and assessing medical students.
  • Evaluating portfolio content validity is crucial for assessing achieved competencies during clerkships.

Purpose of the Study:

  • To evaluate the content validity of portfolios for assessing medical students' competencies during clerkships.
  • To determine if portfolios adequately cover CanMEDS roles.

Main Methods:

  • Reviewed 120 workplace portfolios from three universities.
  • Developed and applied the Validity Inventory for Portfolio Assessment (VIPA) based on CanMEDS roles.
  • Two raters assessed portfolio content sufficiency and interrater agreement using Cohen's Kappa.

Main Results:

  • Portfolios adequately covered communicator (90%) and professional (87%) roles.
  • Coverage for medical expert, collaborator, scholar, and manager roles ranged from 75% to 85%.
  • The health advocate role was underrepresented (59%) with low interrater agreement (Kappa < 0.4).

Conclusions:

  • Portfolios can adequately assess most CanMEDS roles in medical clerkships.
  • The health advocate role requires specific attention for improved representation in portfolios.
  • Portfolio assessment validity is supported, with a noted exception for the health advocate role.
