Colorectal cancer in young adults (YOCR) presents with more advanced symptoms like incontinence and obstruction. These young-onset colorectal cancers are less resectable, though survival rates are similar to older patients.
Area of Science:
- Oncology
- Gastroenterology
- Clinical Research
Background:
- Colorectal cancer (CRC) incidence in younger populations is a subject of ongoing debate.
- Conflicting reports exist regarding the distinct patterns of CRC occurrence and survival in young versus older individuals.
Purpose of the Study:
- To analyze clinicopathological characteristics, treatment, and survival of sporadic young-onset colorectal cancer (YOCR) patients (<40 years).
- To compare YOCR patients with an older cohort (>40 years) to identify differences in disease presentation and outcomes.
Main Methods:
- Retrospective analysis of 172 patients diagnosed with colorectal cancer.
- Comparison of clinicopathological features, treatment modalities, and survival between YOCR (<40 years) and older (>40 years) patient groups.
- Exclusion of patients with hereditary syndromes to focus on sporadic cases.
Main Results:
- YOCR patients (42%) presented more frequently with incontinence (P=0.02) and obstruction (P=0.03).
- A higher proportion of rectal cancers (P=0.01) and left-sided disease was observed in the YOCR group, with significantly lower resectability rates for both left-sided (P=0.04) and rectal cancers (P=0.02).
- Despite differences in presentation and resectability, overall survival rates were similar between the YOCR and older groups.
Conclusions:
- Sporadic colorectal cancers in younger individuals tend to be diagnosed at more advanced stages and are less amenable to surgical resection compared to older patients.
- Further genetic research is warranted to understand the observed left-sided predominance and aggressive nature of sporadic CRC in younger subgroups.
