Changes in bile duct diameter after cholecystectomy: a 5-year prospective study
Ultrasound accurately measured bile duct diameters before and after cholecystectomy. While some minor dilatation occurred post-surgery, significant increases in common hepatic duct and common bile duct size were uncommon.
Area of Science:
- Gastroenterology
- Abdominal Surgery
- Diagnostic Imaging
Background:
- Cholecystectomy is a common surgical procedure.
- Assessing post-operative biliary duct changes is crucial for patient outcomes.
- Ultrasound is a non-invasive imaging modality for evaluating biliary structures.
Purpose of the Study:
- To prospectively evaluate changes in common hepatic duct (CHD) and common bile duct (CBD) diameters after cholecystectomy using ultrasound.
- To compare ultrasound measurements with intraoperative cholangiograms.
- To assess long-term (5-year) changes in ductal dimensions.
Main Methods:
- Prospective ultrasound measurements of CHD and CBD in 24 patients undergoing elective cholecystectomy.
- Comparison of preoperative ultrasound with intraoperative cholangiogram measurements.
- Serial ultrasound follow-up at 1 month, 1 year, and 5 years post-surgery.
Main Results:
- Excellent correlation (r=0.938) between preoperative ultrasound and intraoperative cholangiogram measurements.
- Mean CHD diameter increased slightly from 3.95 mm to 4.48 mm at 5 years (n=21).
- A trend towards minor CBD dilatation was observed post-cholecystectomy, with 7 of 13 measured ducts increasing in size.
Conclusions:
- Ultrasound is a reliable tool for measuring biliary duct diameters.
- Significant dilatation of the common hepatic duct is rare after cholecystectomy.
- A trend towards minor common bile duct dilatation exists post-cholecystectomy, warranting further investigation.
