Summary

The ganglion cell and inner plexiform layer (GCIPL) shows early axonal loss in optic neuritis better than the retinal nerve fiber layer (RNFL). This finding aids in multiple sclerosis diagnosis and neuroprotection trials.

Background:

  • Optic neuritis is crucial for predicting multiple sclerosis (MS) development.
  • Optical coherence tomography (OCT) measures retinal nerve fiber layer (RNFL) for axonal damage in MS.
  • RNFL changes can be masked by inflammation and edema, obscuring early damage.

Purpose of the Study:

  • To compare changes in the ganglion cell and inner plexiform layer (GCIPL) versus RNFL in acute optic neuritis using spectral domain OCT.
  • To assess the sensitivity of GCIPL as an indicator of axonal loss compared to RNFL.

Main Methods:

  • Prospective follow-up of 10 acute optic neuritis patients for 6 months using spectral domain OCT.
  • Comparison with a baseline group of 40 eyes from 20 healthy controls.

Main Results:

  • Affected eyes showed significantly lower GCIPL at onset (p=0.009).
