Summary

Simultaneous robot-assisted adrenalectomy and partial nephrectomy is a safe and feasible procedure for complex renal and adrenal masses. This minimally invasive approach offers ergonomic advantages and enhanced 3D visualization for challenging cases.

Background:

  • Robot-assisted partial nephrectomy and adrenalectomy are established procedures for small renal masses and adrenal tumors, respectively.
  • Robotic surgery enables management of complex cases challenging for traditional laparoscopy.

Observation:

  • A case of simultaneous robot-assisted ipsilateral adrenalectomy and partial nephrectomy for a complex renal cyst (Bosniak IIF) and an estrogen-producing adrenal mass.
  • The procedure involved adrenal tumor excision followed by renal cyst enucleation.

Findings:

  • The simultaneous procedure was completed in 180 minutes with a 20-minute warm ischemia time for renal cyst excision.
  • Histopathology confirmed an adrenocortical adenoma and a benign hemorrhagic renal cyst.
  • The patient was discharged on postoperative day 3 with minimal morbidity.

Implications:

  • Simultaneous ipsilateral adrenalectomy with partial nephrectomy using robotic assistance is feasible and safe.
  • Individualized port placement is crucial based on the small renal mass location.
  • Robotic assistance offers ergonomic benefits and superior 3D visualization compared to laparoscopy for complex procedures.
