Summary

Background:

  • Glaucoma and ocular hypertension are leading causes of vision loss.
  • Effective long-term intraocular pressure (IOP) management is crucial for preventing optic nerve damage.

Purpose of the Study:

  • To evaluate the long-term maintenance effect of topical timolol on intraocular pressure (IOP).
  • To assess treatment outcomes in patients with glaucoma or ocular hypertension receiving timolol monotherapy or combination therapy.

Main Methods:

  • A prospective study involving 155 patients (275 eyes) with glaucoma or ocular hypertension.
  • Mean IOP was calculated from three daytime readings; treatment ranked timolol, timolol plus adjunctive therapy, or surgery.
  • Follow-up averaged 31.6 months.

Main Results:

  • Timolol alone controlled IOP in 63.2% of patients.
  • 23.2% required adjunctive medication, and 13.6% needed laser trabeculoplasty or surgery.
  • Treatment success was higher in ocular hypertension than glaucoma.
  • Treatment failures correlated with higher initial IOP and occurred mostly within 6 months.
  • Adverse effects were transient in 11.2% and serious in 1.9%.

Conclusions:

  • Topical timolol is effective for long-term IOP control in a majority of glaucoma and ocular hypertension patients.
  • Adjunctive therapies are necessary for a significant subset of patients.
  • Higher initial IOP and specific glaucoma types influence treatment outcomes.
