Summary

Statin therapy, used for cardiovascular disease prevention, does not appear to cause tendon rupture. However, simvastatin may actually reduce the risk of tendinopathy.

Background:

  • Statins are widely prescribed for hypercholesterolemia and cardiovascular disease prevention.
  • Concerns exist regarding potential adverse effects of statins on tendons.

Purpose of the Study:

  • To systematically review evidence linking statin therapy to tendinopathy.
  • To assess causality using the Bradford Hill criteria.

Main Methods:

  • Systematic review of randomized controlled trials and observational studies.
  • Searched Ovid MEDLINE, CINAHL Plus, PubMed, and Embase databases.
  • Included English-language studies from January 1966 to October 2015.

Main Results:

  • Four studies (3 cohort, 1 case-control) were included; 3 were high quality.
  • No positive association found between statin use and tendon rupture in the general population.
  • Evidence suggests simvastatin may reduce tendinopathy risk.

Conclusions:

  • Limited evidence implicates statin therapy as a cause of tendon rupture.
  • Simvastatin shows potential protective effects against tendinopathy.
