Heparin is preferred for cardiopulmonary bypass (CPB), but alternatives are needed for patients with heparin-induced thrombocytopenia (HIT). Direct thrombin inhibitors, like bivalirudin, are recommended as safe and effective CPB anticoagulants in these cases.
