Summary

The manual provocation test (MPT) effectively diagnoses intermittent involutional entropion of the lower eyelid. This straightforward clinical evaluation aids in identifying patients needing surgical correction for this condition.

Area of Science:

  • Ophthalmology
  • Oculoplastics
  • Surgical Innovation

Background:

  • Intermittent involutional entropion presents diagnostic challenges due to variable presentation.
  • Patients often experience ocular irritation without apparent eyelid malposition during examination.

Purpose of the Study:

  • To introduce and describe the manual provocation test (MPT) for diagnosing intermittent involutional entropion of the lower eyelid.
  • To evaluate the efficacy of the MPT in a clinical setting.

Main Methods:

  • Retrospective and prospective evaluation of the MPT in patients with suspected intermittent entropion.
  • The MPT involves grasping the lower eyelid, drawing it anteriorly, forceful eyelid closure, and observing for entropion.
  • Comparison with an age-matched control group undergoing blepharoplasty.

Main Results:

  • The MPT successfully elicited entropion in all 13 eyelids of patients with intermittent involutional entropion.
  • Surgical intervention in 9 patients resulted in successful symptom improvement and a negative MPT post-operatively.
  • No positive MPT results were observed in the 20 control patients.

Conclusions:

  • The manual provocation test (MPT) is a valuable and simple diagnostic tool for intermittent lower eyelid entropion.
  • The MPT aids in the clinical evaluation and management of patients with this condition.
