This study shows that incorporating plaque microvasculature geometry improves the accuracy of intravascular ultrasound elastography for predicting atherosclerotic plaque rupture risk.
Area of Science:
- Biomedical Engineering
- Cardiovascular Imaging
- Medical Ultrasound
Background:
- Model-based intravascular ultrasound elastography estimates stress distribution in vascular tissue to predict atherosclerotic plaque rupture.
- Concerns exist that plaque microvasculature may reduce the accuracy of these stress estimations.
Purpose of the Study:
- To investigate the impact of plaque microvasculature on model-based elastography performance.
- To develop a contrast-enhanced intravascular ultrasound system for this investigation.
Main Methods:
- Simulations using microvessel diameters of 200, 400, and 800 μm.
- Phantom studies with a 750 μm microvessel.
- Comparison of stress recovery with and without a priori microvessel geometry information.
Main Results:
- Peak stress errors in the fibrous cap were 3% when microvessel geometry was incorporated.
- Peak stress errors reached 38% without microvessel geometry information.
- Contrast-enhanced ultrasound imaging significantly improved accuracy.
Conclusions:
- Incorporating plaque microvasculature geometry enhances the accuracy of stress distribution estimates.
- This improvement is crucial for reliable prediction of atherosclerotic plaque rupture.
- Contrast-enhanced ultrasound is a valuable tool for this application.
