Background:

  • Neuroblastoma is a common pediatric solid tumor, particularly in infants.
  • Current staging systems include the International Neuroblastoma Staging System (INSS) and the International Neuroblastoma Risk Group Staging System (INRGSS).
  • The INRGSS utilizes image-defined risk factors (IDRFs) as indicators of surgical risk.

Purpose of the Study:

  • To investigate the correlation between IDRFs and surgical outcomes in neuroblastoma patients.
  • To assess the relationship between IDRFs and surgical complications, radical resection, local recurrence, and overall survival.

Main Methods:

  • Retrospective analysis of 102 neuroblastoma patients undergoing surgery between 2003 and 2010.
  • Evaluation of medical records for IDRF status, surgical complications, extent of tumor removal, local recurrence, and overall survival.

Main Results:

  • 86 out of 102 patients had one or more IDRFs.
  • 21% of patients experienced intra- or postoperative complications, predominantly in the IDRF-positive group.
  • IDRF status correlated inversely with macroscopical tumor removal (p=0.001) and likelihood of radical resection (p<0.001).
  • IDRF-positive patients had a higher incidence of local recurrence (p=0.037) and decreased overall survival (p=0.011).

Conclusions:

  • Image-defined risk factors (IDRFs) are significant predictors of surgical complications and outcomes in neuroblastoma.
  • IDRFs are valuable for risk stratification and surgical planning in pediatric neuroblastoma cases.
  • Consideration of IDRFs by pediatric surgeons can optimize neuroblastoma treatment strategies.
