[Permeability changes in bronchiolo-alveolar epithelium].

    Summary

Pulmonary epithelial permeability, measured by 99mTc-DTPA clearance, is increased in smokers and interstitial lung disease patients. This lung permeability increase is reversible in smokers after cessation.

    Area of Science:

    • Pulmonary Medicine
    • Radiopharmacology
    • Cell Biology

    Context:

    • Assessing lung pathophysiology in smokers and interstitial lung diseases (ILDs).
    • Investigating epithelial permeability in the bronchiolo-alveolar region.
    • Utilizing 99mTc-DTPA (diethylene triamine penta acetate) clearance as a measure of pulmonary epithelial permeability.

    Purpose:

    • To evaluate changes in pulmonary epithelial permeability in healthy smokers and patients with ILDs.
    • To determine if increased lung epithelial permeability is a common feature in these conditions.
    • To assess the sensitivity of the 99mTc-DTPA clearance method for detecting lung inflammation.

    Summary:

    • Pulmonary epithelial permeability, quantified by the rate constant (kep) of inhaled 99mTc-DTPA clearance, was assessed.
    • Healthy smokers (53%) and patients with ILDs exhibited increased kep values compared to healthy non-smokers.
    • Increased permeability in young male smokers correlated with cigarette consumption and was reversible upon smoking cessation.

    Impact:

    • The 99mTc-DTPA clearance method is a sensitive indicator of inflammatory changes in the bronchiolo-alveolar epithelium.
    • Findings suggest smoking-induced lung injury affects epithelial permeability, which may be reversible.
    • Highlights potential for this method in early detection and monitoring of lung diseases.
