    Wolf Parkinson White syndrome, a pre-excitation disorder, can manifest as irregular wide complex tachycardia, potentially leading to sudden cardiac death. Early recognition and appropriate management, avoiding AV nodal blockers, are crucial.

    Background:

    • Wolf Parkinson White (WPW) syndrome is a cardiac pre-excitation disorder caused by an accessory electrical conduction pathway.
    • Standard electrocardiography (ECG) in WPW syndrome typically shows a short PR interval, prolonged QRS duration, and delta waves during normal sinus rhythm.

    Observation:

    • Atrial fibrillation in patients with WPW syndrome can present as irregular wide complex tachycardia.
    • This presentation can be the initial sign of the underlying conduction abnormality, particularly in younger individuals.
    • Irregular wide complex tachycardia carries a risk of precipitating ventricular fibrillation and sudden cardiac death.

    Findings:

    • Emergency management for hemodynamically unstable patients with irregular wide complex tachycardia includes synchronized cardioversion.
