Aneurysmal bone cysts in the cervical spine are challenging. Transoral vertebroplasty successfully treated a C2 lesion in a young patient, offering a safe and effective stabilization method.

- Imaging revealed an extensive C2 vertebral body lytic lesion consistent with an ABC.

- The procedure was technically successful, with no intraoperative or postoperative complications.
- Histological analysis of the cyst aspirate showed no evidence of tumoral cells.
Implications:
- Transoral vertebroplasty is a safe and effective minimally invasive option for stabilizing C2 aneurysmal bone cysts.
- This technique can alleviate symptoms and prevent catastrophic vertebral collapse.
- ABCs should be considered in the differential diagnosis of C2 vertebral lytic lesions.
