Diagnosing malignant pleural effusions is challenging. This study identified protein biomarkers in pleural fluid to differentiate between mesothelioma, lung adenocarcinoma, lymphoma, and tuberculosis (TB), improving diagnostic accuracy.

Background:

Purpose of the Study:

  • To identify protein biomarkers in pleural fluid for distinguishing between mesothelioma, lung adenocarcinoma, lymphoma, and tuberculosis (TB).

Main Methods:

  • A multiplex protein biochip assay was used to analyze pleural fluid from patients with mesothelioma, lung adenocarcinoma, lymphoma, and TB.
  • Candidate biomarkers were validated using commercial techniques in an independent patient cohort.
  • Statistical models were employed to assess the discriminatory power of identified biomarkers.

Main Results:

  • A panel of four proteins (metalloproteinase-9, cathepsin-B, C-reactive protein, chondroitin sulfate) effectively differentiated malignant from TB effusions (85% sensitivity, 100% specificity).
  • A three-protein panel (CA19-9, CA15-3, kallikrein-12) distinguished lung adenocarcinoma from mesothelioma (65% sensitivity, 100% specificity).
  • Cathepsin-B combined with patient age showed high accuracy in differentiating lymphoma from TB effusions (72% sensitivity, 100% specificity).

Conclusions:

  • Specific protein panels in pleural fluid can accurately diagnose malignant effusions and differentiate between specific cancer types and TB.
  • These protein biomarkers offer a promising tool to improve the diagnostic accuracy of challenging pleural effusion cases.
  • Cathepsin-B, in conjunction with patient age, aids in distinguishing lymphomatous from tuberculous effusions.
