Chronic Kidney Disease III: Interprofessional Care01:28

Chronic Kidney Disease III: Interprofessional Care

595
Chronic kidney disease (CKD) requires collaborative and comprehensive management. CKD progresses through stages and can lead to end-stage kidney disease (ESKD) if untreated. Interprofessional collaboration and patient education are crucial, enabling patients to manage their health and improve their quality of life.Diagnostic approach for chronic kidney diseaseThe diagnosis of CKD primarily focuses on the glomerular filtration rate (GFR), which assesses kidney function by measuring how well...

Nephrotic Syndrome I : Introduction

950
Nephrotic Syndrome is a chronic kidney disorder defined by clinical findings such as severe proteinuria, hypoalbuminemia, hyperlipidemia, and edema. These symptoms result from damage to the glomeruli, the kidney’s filtering units, increasing their permeability to proteins.Definition and Meaning:Proteinuria, defined as the loss of more than 3.5 grams of protein per day in adults, is a crucial feature of nephrotic syndrome. This condition is often accompanied by edema, the accumulation of...

Renal Corpuscle01:20

Renal Corpuscle

8.5K
The glomerulus and Bowman's capsule are two essential components of the nephron, which is the functional unit of the kidney. These microscopic structures play a critical role in the process of blood filtration to produce urine.
Glomerulus: Structure and Function
The glomerulus is a tiny, intricate network of capillaries located at the beginning of the nephron. It's enveloped by the Bowman's capsule and receives its blood supply from an afferent arteriole, which divides into numerous...

Acute Kidney Injury II: Pathophysiology

1.8K
Acute kidney injury (AKI) causes are categorized into three primary categories based on the location of the injury: prerenal, intrarenal (or intrinsic), and postrenal causes. This classification guides clinical management and illustrates how different pathways can impair kidney function.Etiology and Pathophysiology of Acute Kidney Injury1. Prerenal causesEtiology: Prerenal Acute Kidney Injury, the most common type, occurs when reduced blood flow to the kidneys decreases filtration capacity...

Drug Dosing in Renal Diseases: Measurement of Glomerular Filtration Rate

100
The glomerular filtration rate (GFR) is a critical indicator of kidney health, reflecting how well the kidneys filter blood. Changes in GFR can signal potential kidney impairment, necessitating accurate measurement methods to monitor kidney function effectively.Various molecules can serve as markers for GFR measurement, with the ideal marker meeting several specific criteria. It must freely filter at the glomerulus, avoid reabsorption or secretion by the renal tubules, remain unmetabolized, not...

Nephrons01:10

Nephrons

8.4K
The kidneys are intricate organs with millions of working units known as nephrons. Each nephron features two major structures: the renal corpuscle, which facilitates blood plasma filtration, and the renal tubule, which handles the glomerular filtrate. Blood supply is directly linked to the nephrons. The renal corpuscle consists of the glomerulus, a capillary network, and the Bowman's capsule, a double-walled epithelial structure that encases the glomerulus. The filtering of blood plasma...

Area of Science:

  • Nephrology
  • Rheumatology
  • Immunology

Background:

  • Immunoglobulin G4-related disease (IgG4-RD) is a multi-organ fibroinflammatory condition.
  • Renal involvement in IgG4-RD includes tubulointerstitial nephritis (TIN) and membranous glomerulonephropathy.
  • Retroperitoneal fibrosis (RPF) is a common IgG4-related condition that can cause kidney obstruction.

Purpose of the Study:

  • To evaluate the efficacy of an intensified immunosuppressive protocol for aggressive IgG4-related kidney disease.
  • To assess treatment outcomes for IgG4-related retroperitoneal fibrosis.

Main Methods:

  • Utilized contrast-enhanced computerized tomography, magnetic resonance imaging, and PET/CT for diagnosis and monitoring.
  • Administered an intensified immunosuppressive protocol to patients with aggressive IgG4-related TIN and IgG4-related RPF.
  • Monitored patients for treatment response and adverse effects.

Main Results:

  • Successful treatment of three cases of aggressive IgG4-related TIN.
  • Successful treatment of two cases of IgG4-related RPF.
  • Achieved good therapeutic outcomes without severe adverse effects.

Conclusions:

  • An intensified immunosuppressive protocol is effective for aggressive IgG4-related kidney disease.
  • This protocol offers a viable treatment option for IgG4-related RPF.
  • Aggressive immunosuppression can be safely used in managing complex IgG4-RD cases.
