    Summary

    Transrectal ultrasonography (TRUS)-guided prostate biopsy pain can be managed. Periprostatic anesthesia with lidocaine effectively reduced pain without increasing complications.

    Background:

    • Prostate biopsy using transrectal ultrasonography (TRUS) is crucial for prostate cancer diagnosis.
    • Patient fear of pain associated with TRUS-guided prostate biopsy can deter them from undergoing the procedure.

    Purpose of the Study:

    • To compare the efficacy of different anesthesia methods for TRUS-guided prostate biopsy.
    • To identify the optimal pain management strategy for this diagnostic procedure.

    Main Methods:

    • Four groups of 40 patients each were studied.
    • Group 1: TRUS-guided periprostatic anesthesia with 1% lidocaine.
    • Group 2: Intrarectal EMLA cream (lidocaine 2.5%, prilocaine 2.5%).
    • Group 3: Intrarectal 10% lidocaine spray.
    • Group 4: Placebo (ultrasonic gel).
    • Pain was assessed using a 100-score linear visual analog scale (LVS) and a 5-score numeric visual scale (NVS).

    Main Results:

    • Patients receiving TRUS-guided periprostatic anesthesia with 1% lidocaine reported the minimal pain scores.
    • No significant increase in complications was observed with this anesthesia method.
    • EMLA cream and lidocaine spray also showed some pain reduction compared to placebo, but were less effective than periprostatic anesthesia.

    Conclusions:

    • TRUS-guided periprostatic anesthesia with 1% lidocaine is the most effective method for managing pain during prostate biopsy.
    • This anesthesia technique is safe and does not increase the risk of complications.
    • Effective pain control can alleviate patient anxiety and improve adherence to prostate cancer diagnostic procedures.
