Observation:

  • A 28-year-old man presented with progressive, unilateral weakness and atrophy of the left shoulder girdle and deltoid muscles.
  • Neurological exam revealed atrophy and weakness in the left deltoid, infraspinatus, and supraspinatus.
  • EMG showed active/chronic neurogenic patterns in C5-C6 myotomes; cervical spine MRI was normal.

Findings:

  • The patient's presentation represented a rare proximal upper limb form of monomelic amyotrophy.
  • Conservative treatment yielded positive outcomes for the patient.

Implications:

  • Recognizing this rare variant of monomelic amyotrophy is crucial for differential diagnosis.
  • Accurate diagnosis prevents misattribution to conditions like amyotrophic lateral sclerosis, improving patient management and prognosis.
