Abstract

Insights

Anaplastic Lymphoma Kinase (ALK) positive non-small cell lung cancer patients with poor performance status can benefit from alectinib treatment. This therapy demonstrated significant improvement in a patient with severe respiratory failure.

Area of Science:

  • Oncology
  • Molecular Biology
  • Pharmacology

Background:

  • Anaplastic Lymphoma Kinase (ALK) rearrangements drive certain non-small cell lung cancers (NSCLC).
  • ALK tyrosine kinase inhibitors (TKIs) show efficacy in ALK-positive NSCLC.
  • Patients with poor performance status (3 or 4) have been excluded from ALK TKI clinical trials.

Observation:

  • A 52-year-old patient with ALK-positive NSCLC presented with performance status 4 due to severe respiratory failure.
  • The patient received alectinib as first-line therapy.

Findings:

  • Alectinib treatment resulted in a dramatic response.
  • The patient's performance status improved significantly from 4 to 1.
  • No severe adverse events were observed during treatment.

Implications:

  • Alectinib is a viable therapeutic option for ALK-positive NSCLC patients with poor performance status.
  • This case expands the potential use of ALK inhibitors to a previously underrepresented patient group.
