Background:

  • Rigid bronchoscopy is a key procedure for managing central airway disorders.
  • A standardized tool for assessing operator competency in rigid bronchoscopy was lacking.
  • The Rigid Bronchoscopy Tool for Assessment of Skills and Competence (RIGID-TASC) was developed for objective skill evaluation.

Purpose of the Study:

  • To validate the RIGID-TASC's ability to distinguish skill levels among novice, intermediate, and expert rigid bronchoscopy operators.
  • To assess the interrater reliability of the RIGID-TASC when used by independent testers.

Main Methods:

  • 30 physicians (10 novice, 10 intermediate, 10 expert) performed rigid bronchoscopic intubation and navigation on a manikin.
  • Participants were selected from pulmonary, critical care, and interventional pulmonology training programs.
  • Two independent testers scored each performance using the RIGID-TASC.

Main Results:

  • Mean RIGID-TASC scores significantly differed across skill groups: novice (58.10), intermediate (78.15), and expert (94.40).
  • Statistically significant differences were found between novice and intermediate (P=0.001) and intermediate and expert (P=0.008) operators.
  • High interrater reliability was demonstrated, with an intraclass correlation coefficient of 0.95 (95% CI = 0.90-0.98).

Conclusions:

  • The RIGID-TASC demonstrates construct validity and high interrater reliability for assessing basic rigid bronchoscopic skills.
  • This tool can objectively and reliably classify operator competency from novice to expert levels.
  • RIGID-TASC is a valuable instrument for standardizing training and competency evaluation in rigid bronchoscopy.
