Observation:

  • A 34-year-old male patient with type 1 diabetes mellitus underwent 130 days of ocular wavefront and blood glucose monitoring post-insulin initiation.
  • Measurements included refraction, higher-order aberrations using a Hartmann-Shack sensor, and frequent blood glucose readings.
  • Significant bilateral hyperopic shifts followed by myopic shifts were recorded, peaking around 15-18 days after insulin treatment began.

Findings:

  • Equivalent sphere showed a 5 D hyperopic shift, then a 5 D myopic shift, stabilizing after 60 days.
  • Higher-order aberrations, primarily spherical aberration, also exhibited transient changes correlating with refractive shifts.
  • Cylinder values remained stable, while Zernike coefficient c12 for spherical aberration increased substantially before returning to baseline.

Implications:

  • Wavefront aberrometry provides quantitative data on transient ocular optical changes during type 1 diabetes treatment.
  • The observed transient refractive shifts suggest alterations in the crystalline lens, potentially its refractive index.
  • Further research is needed to fully elucidate the mechanisms behind these lens-based optical changes in diabetes.
