This study compared methods for calculating limb volume in hyperthermic isolated limb perfusion (HILP). Computed tomography (CT) based methods, particularly Osirix and Oncentra Masterplan, offer accurate and efficient alternatives to traditional water displacement volumetry.

Area of Science:

  • Oncology
  • Medical Imaging
  • Surgical Oncology

Background:

  • Hyperthermic isolated limb perfusion (HILP) is an alternative to amputation for extremity sarcomas and melanomas.
  • Drug dosage in HILP relies on accurate limb volume measurement.
  • Traditional water displacement volumetry (WDV) is time-consuming and difficult for certain patient groups.

Purpose of the Study:

  • To compare existing methods for regional tissue volume calculation in HILP.
  • To validate open-source software for limb volume assessment in HILP.

Main Methods:

  • A review of 22 patients with lower limb melanoma or sarcoma.
  • Limb volume calculated using WDV, tape measurements, and CT image segmentation (Osirix, Oncentra Masterplan).

Main Results:

  • Overall concordance correlation coefficient (CCC) of 0.92 for whole limb volume calculations.
  • Near-perfect concordance (CCC = 0.99) between Osirix and Oncentra.
  • CT methods allow for reliable segmentation and tissue-specific volume analysis (bone, fat, water).

Conclusions:

  • CT-based methods provide accurate and efficient limb volume calculations for HILP.
  • Osirix and Oncentra Masterplan are validated tools for HILP volume assessment.
  • CT enables detailed tissue composition analysis, aiding treatment planning.
