Summary

Three orthodontic retention methods showed similar long-term effectiveness. Removable vacuum-formed retainers (VFR) with canine-to-canine retainers, VFR with tooth stripping, and prefabricated positioners all yielded favorable results five years post-treatment.

Purpose of the Study:

  • To compare the long-term clinical effectiveness of three different orthodontic retention strategies.

  • A single-center randomized controlled trial involving 75 patients.
  • Three groups: removable vacuum-formed retainer (VFR) with bonded canine-to-canine retainer, VFR with mandibular anterior tooth stripping, and prefabricated positioner.
  • Outcome measures included Little's irregularity index, arch dimensions, and overbite/overjet, assessed five years post-retention.

Main Results:

  • 69 patients completed two years, and 49 were available at five years post-retention.
  • No statistically significant differences were observed between the three retention groups in any measured outcome.
  • All methods demonstrated comparable clinical success in maintaining orthodontic results.

Conclusions:

  • Maxillary VFR with a mandibular bonded canine-to-canine retainer, maxillary VFR with mandibular anterior tooth stripping, and prefabricated positioners are all effective retention options.
  • These methods provide equally favorable clinical outcomes five years after orthodontic treatment.
  • Clinicians can confidently recommend any of these three retention approaches.
