Background:

Liquid silicone is a popular, inexpensive soft-tissue filler for cosmetic augmentation.
Complications like infection and migration are reported, but granulomatous reactions are less common.
Procedures performed in developing countries may carry increased risks due to nonmedical-grade silicone use.

Observation:

A 54-year-old woman presented with buttock pain and inflammation one year after silicone buttock augmentation abroad.
She was diagnosed with multiple silicone granulomas, an unusual inflammatory complication.
Symptoms resolved completely after a three-week course of minocycline.

Findings:

Granulomatous reactions to liquid silicone injections can manifest months to years post-procedure.
