Summary

Nipple excision during nipple-sparing mastectomy (NSM) is sometimes necessary. Intraoperative frozen section analysis aids detection, and subsequent nipple reconstruction offers excellent aesthetic outcomes for patients.

Background:

  • Nipple-sparing mastectomy (NSM) may require nipple excision due to occult malignancy, detected in 0-58% of cases.
  • This study reviews nipple resection following attempted NSM and subsequent reconstruction outcomes.

Purpose of the Study:

  • To evaluate the efficacy of intraoperative frozen section in detecting occult nipple malignancy during NSM.
  • To assess the outcomes of nipple reconstruction after excision.

Main Methods:

  • Retrospective review of 568 attempted NSMs with immediate implant-based reconstruction (July 2006-April 2015).
  • Identification of patients who underwent nipple excision, analysis of indications, pathology reports, and reconstructive results.

Main Results:

  • 34 (6%) cases required nipple excision; indications included positive frozen sections (53%), positive permanent sections (29%), clinical suspicion (3%), and patient request/symmetry (15%).
  • Frozen section sensitivity was 64% (75% including suspicious/atypical findings), with no false positives. Residual disease was found in 56% of positive frozen section cases and 30% of positive permanent section cases.
  • Nipple reconstruction was performed in 68% of cases, with excellent aesthetic results in 83% using CV flap, skate flap, or nipple-sharing techniques.

Conclusions:

  • Intraoperative subareolar frozen section is beneficial for detecting occult nipple disease during NSM.
  • Nipple reconstruction following excision can yield excellent aesthetic results, improving patient satisfaction.
