Methods of Documentation VI: Case Management Model

1.0K
The case management model is a multidisciplinary approach that involves healthcare professionals from diverse disciplines, such as physicians, nurses, therapists, social workers, and pharmacists, working collaboratively to address the various needs of patients. Each healthcare professional brings unique expertise and perspectives, contributing to a more comprehensive understanding of the patient's condition and tailoring treatment plans accordingly.
For example, a patient with a chronic...

Methods of Documentation II: POMR

1.6K
The Problem-Oriented Medical Record (POMR) revolutionized medical record-keeping by introducing a systematic approach focusing on the patient's problems rather than merely listing symptoms. Dr. Lawrence Weed's introduction of this method in the 1960s marked a significant advancement in medical documentation. The POMR framework consists of four key components: the database, problem list, plan of care, and progress notes.

Planning Nursing Care I01:21

Planning Nursing Care I

6.3K
The planning phase of the nursing process helps nurses set priorities, outline patient-centered goals and expected outcomes, and tailor nursing interventions to align with the aligned care plan. Through the planning phase, the nurse applies critical thinking skills to align and develop interventions according to the patient's needs. It provides continuity of care allowing patients to receive the maximum benefit from treatment. It serves as a pilot plan for allocating individual staff to a...

Nursing Implementation01:15

Nursing Implementation

6.5K
Implementation is the execution of the nursing care plan developed during the planning phase.
The five steps to implementing effective nursing care include reassessing the patient, reviewing and revising the existing nursing care plan, organizing the resources and care delivery, anticipating and preventing complications, and implementing nursing interventions.

Social Loafing01:37

Social Loafing

39.8K
Another way in which a group presence can affect performance is social loafing—the exertion of less effort by a person working together with a group. Social loafing occurs when our individual performance cannot be evaluated separately from the group. Thus, group performance declines on easy tasks (Karau & Williams, 1993). Essentially individual group members loaf and let other group members pick up the slack. Because each individual’s efforts cannot be evaluated,...

Area of Science:

  • Gerontology
  • Health Services Research
  • Nursing Practice

Background:

  • Case management is a standard practice in diverse healthcare settings.
  • Understanding time allocation and activity frequency is crucial for optimizing community aged care.

Purpose of the Study:

  • To examine time allocation to core functions by community aged care case managers.
  • To determine the frequency of specific case management activities.
  • To identify factors influencing the frequency of these activities.

Main Methods:

  • A survey of 154 case managers in Victoria, Australia (17.1% response rate).
  • Data collected on socio-demographics, time allocation to six functions, and frequency of 41 activities.
  • Ordinal regression analysis used to identify influencing factors.

Main Results:

  • Care coordination received the largest time proportion (22.0%), outcome evaluation the smallest (8.0%).
  • Over 70% of activities were rated as high frequency, with outcome evaluation and needs assessment activities performed less frequently.
  • Emphasis on case management goals and longer work experience were associated with higher frequency of specific activities, particularly outcome evaluation.

Conclusions:

  • Community aged care case managers exhibit a uniform practice pattern in activity performance.
  • The less frequent performance of outcome evaluation activities warrants further investigation regarding expected standards.
