Purpose of the Study:

  • To investigate the incidence of very frequent exacerbations (≥3 per year) in COPD patients.
  • To identify key risk factors associated with very frequent exacerbations in this population.

Main Methods:

  • A 2-year prospective cohort study involving 512 primary care patients with COPD.
  • Exacerbations were defined as events requiring antibiotics/systemic steroids or hospital admission.
  • Statistical analysis using odds ratios (OR) to determine associated factors.

Main Results:

  • High incidence rates of exacerbation were observed: 61.7% in year 1 and 63.9% in year 2.
  • Factors associated with very frequent exacerbations in year 1 included prior hospital admission (OR 1.69) and dyspnea severity (OR 2.86-5.83).
  • In year 2, female sex (OR 4.17), previous hospital admissions (OR 2.90), smoking status (OR 2.00), and reduced forced vital capacity (OR 0.98) were significant factors.

Conclusions:

  • The study confirms a high incidence of exacerbations among COPD patients.
  • Previous hospital admission for an exacerbation is a robust predictor, identifying patients at elevated risk.
