Background:

  • Bone sarcoidosis is a rare manifestation of sarcoidosis, frequently associated with pulmonary involvement.

  • Understanding the prevalence and patterns of bone involvement is crucial for managing chronic sarcoidosis.

Purpose of the Study:

  • To determine the prevalence of bone involvement in active chronic sarcoidosis.
  • To identify the distribution patterns of osseous lesions using FDG PET/CT.
  • To compare the diagnostic utility of FDG PET/CT with conventional CT for bone sarcoidosis.

Main Methods:

  • 98 patients with active chronic sarcoidosis underwent FDG PET/CT, MDCT, and serum ACE level assessment.
  • 82 patients with confirmed active disease were screened for bone sarcoidosis using FDG PET/CT.
  • Analysis focused on FDG uptake patterns and CT findings in osseous structures.

Main Results:

  • Bone sarcoidosis was identified in 18 out of 82 (22%) patients with active chronic sarcoidosis.
  • Osseous involvement was most common in the pelvis (61.1%), vertebrae (44.4%), and ribs (27.8%).
  • FDG PET/CT detected abnormalities in 22% of patients, while conventional CT identified osseous involvement in only 5%.

Conclusions:

  • FDG PET/CT reveals a significant prevalence of bone involvement (22%) in active chronic sarcoidosis.
  • Osseous abnormalities detected by FDG PET/CT are frequently missed by standard CT imaging.
  • FDG PET/CT is a valuable tool for diagnosing and assessing the extent of bone sarcoidosis.
