Summary

Retro-rectal cystic hamartoma, or tailgut cyst, is a rare congenital lesion. Complete surgical excision is crucial for a favorable prognosis, aided by advanced imaging and multidisciplinary surgical collaboration.

Background:

  • Retro-rectal cystic hamartoma (tailgut cyst) is an uncommon congenital lesion in the retro-rectal space.
  • Diagnosis and surgical approach are challenging due to the unfamiliarity of the retro-pelvic space.

Observation:

  • A 51-year-old woman presented with perianal paresthesia and pain.
  • MRI revealed a retropelvic lesion with specific signal intensities; CT showed a sacral bony defect.

Findings:

  • Histopathology confirmed a multiloculated cyst with mucoid material, lined by squamous and glandular mucinous epithelium.
  • Successful surgical removal was achieved via a posterior approach.

Implications:

  • Complete surgical excision is essential for a good prognosis in tailgut cyst cases.
  • Preoperative high-resolution imaging and collaboration between neurosurgeons and general surgeons enhance diagnostic accuracy and surgical safety.
