Summary

Electrocardiogram changes mimicking heart attack can occur after subarachnoid hemorrhage. These transient ST-segment abnormalities are likely due to autonomic nervous system effects on the heart, not primary heart disease.

Background:

  • Subarachnoid hemorrhage (SAH) is a serious neurological condition.
  • Electrocardiographic (ECG) changes are sometimes observed in SAH patients.
  • Distinguishing cardiac from non-cardiac ECG changes is crucial for patient management.

Observation:

  • Two patients with SAH presented with ECG findings simulating acute myocardial infarction.
  • These included ST-segment elevation and electrical alternans.
  • No underlying heart disease was detected in either patient.

Findings:

  • The observed ST-segment changes were transient.
  • No persistent ECG abnormalities indicative of myocardial damage or ischemia were noted.
  • The ECG manifestations were attributed to SAH, not primary cardiac pathology.
