Summary

Casein glycomacropeptide (CGMP) supplementation showed similar effectiveness to increased mesalamine dosage in treating active ulcerative colitis (UC). This nutritional therapy was safe and well-tolerated by patients with active distal UC.

Background:

  • Ulcerative colitis (UC) is a chronic inflammatory bowel disease.
  • Dietary supplements are explored for their anti-inflammatory potential in managing UC.
  • Casein glycomacropeptide (CGMP), a whey protein fraction, was investigated for its effects in active UC.

Purpose of the Study:

  • To evaluate the efficacy and safety of CGMP as a nutritional therapy in patients with active ulcerative colitis.
  • To compare the effects of CGMP supplementation with dose escalation of standard mesalamine treatment.
  • To assess clinical, endoscopic, and biomarker responses in active distal UC.

Main Methods:

  • A randomized open-label study involving 24 patients with active distal UC.
  • Participants received either standard treatment plus CGMP (30g/day) or escalated dose of oral mesalamine (4800mg/day) for 4 weeks.
  • Disease activity was monitored using clinical (SCCAI), endoscopic, mucosal, and circulating markers.

Main Results:

  • CGMP supplementation resulted in similar clinical improvements (SCCAI changes) compared to mesalamine dose escalation.
  • No significant differences were observed in disease extent, severity, or SCCAI reduction between the CGMP and mesalamine groups.
  • CGMP was well-tolerated and accepted by patients, with good adherence documented up to 8 weeks.

Conclusions:

  • CGMP is a safe and well-accepted nutritional therapy for patients with active distal UC.
  • The addition of CGMP to standard treatment demonstrated comparable disease-modifying effects to mesalamine dose escalation.
  • CGMP represents a potential therapeutic option for managing active ulcerative colitis.
