Background:

  • Merkel cell carcinoma (MCC) is a rare, aggressive skin cancer with a poor prognosis.
  • Radical local excision and radiotherapy are essential for curative treatment of MCC.
  • Plastic surgery is often required to manage defects and ensure optimal oncologic outcomes.

Observation:

  • Fifteen patients with MCC underwent treatment between 2003 and 2016.
  • Complete tumor excision was achieved in 13 patients, with a 2 cm safety margin in most cases.
  • Various reconstruction methods, including primary suturing, skin grafts, and free flaps, were employed.

Findings:

  • All reconstructed defects showed good and stable results, even with adjuvant radiotherapy.
  • Complex reconstruction methods, like free flap transplantation, effectively managed extensive tumor resections.

Implications:

  • Aggressive local excision, guided by modern plastic surgery techniques, is recommended for MCC.
  • Interdisciplinary treatment enhances curative potential and reconstructive safety for MCC patients, regardless of tumor extent or location.
