Summary

Laparoscopic aortic surgery, even by trained teams, shows a higher risk of adverse events (AEs) post-operation and long-term compared to conventional open surgery. Coronary artery disease also independently increases this risk.

Purpose of the Study:

  • To compare post-operative and mid-term outcomes between laparoscopic and conventional open aortic surgery.
  • To evaluate the safety and efficacy of laparoscopic aortic repair in a trained surgical team setting.
  • To identify risk factors associated with adverse events in aortic surgery.

Main Methods:

  • A prospective study of 228 patients undergoing aortic bypass surgery (abdominal aortic aneurysm or occlusive aorto-iliac disease) from 2006-2011.
  • Patients were divided into conventional open surgery (n=145) and total laparoscopic repair (n=83) groups.
  • Propensity score matching was used to create comparable groups, followed by univariate and multivariate analyses (logistic regression, Cox model) of adverse events.

  • Coronary artery disease is a significant independent risk factor for adverse events in both early and late post-operative periods.
  • Further research with larger cohorts may be needed to clarify specific risks for aneurysmal versus occlusive aortic disease.
