Summary

Tetrodotoxin (TTX) from pufferfish poisoning equally impacts nerve fibers, causing reversible conduction slowing. Symptoms rapidly improved as TTX levels decreased, showing a direct link between toxin and nerve function.

Background:

  • Tetrodotoxin (TTX) poisoning, often from pufferfish, can cause neurological symptoms.
  • Nerve conduction studies are crucial for diagnosing peripheral nerve disorders.

Observation:

  • Serial nerve conduction studies were performed on a patient with pufferfish-induced tetrodotoxin poisoning.
  • Both muscle and sensory nerve action potential velocities and amplitudes were equally affected.

Findings:

  • No temporal dispersion or focal conduction block was observed.
  • Proximal motor (F wave) latencies were prolonged.
  • Abnormalities rapidly resolved alongside clinical improvement and reduced urinary TTX excretion.

Implications:

  • Tetrodotoxin reversibly affects myelinated nerve fibers uniformly along the axon.
