Summary

Tonsillectomy significantly improved obstructive sleep apnea (OSA) in adults with large tonsils, reducing the apnea-hypopnea index and daytime sleepiness. This surgical intervention offers a promising treatment option for this patient group.

Background:

  • Obstructive sleep apnea (OSA) is a common sleep disorder.
  • Large tonsils can contribute to OSA in adults.
  • Limited evidence exists on tonsillectomy's efficacy for adult OSA with large tonsils.

Purpose of the Study:

  • To evaluate the effectiveness of tonsillectomy in treating adult OSA patients with large tonsils.
  • To assess the impact of tonsillectomy on apnea-hypopnea index (AHI) and daytime sleepiness.

Main Methods:

  • A multicenter prospective interventional study.
  • 28 adult patients (18-59 years) with OSA (AHI > 10) and large tonsils (Friedman size 3-4) were included.
  • Primary outcome: AHI measured 6 months post-surgery. Secondary outcomes: Epworth Sleepiness Scale (ESS) and swallowing function.

Main Results:

  • Mean AHI decreased from 40 to 7 units/hour (P < 0.001), with 64% of patients cured.
  • Mean ESS scores reduced significantly, indicating decreased daytime sleepiness (P < 0.001).
  • Swallowing function improved in 5 of 8 patients with pre-existing dysfunction.

Conclusions:

  • Tonsillectomy is an effective treatment for adult obstructive sleep apnea patients with large tonsils.
  • The procedure leads to significant improvements in sleep apnea severity and quality of life.
  • Tonsillectomy should be considered for adults suffering from OSA and enlarged tonsils.
