Purpose of the Study:

  • To gather validity evidence for assessing mastery learning using a VR transabdominal ultrasound simulator.
  • To determine if simulator metrics can differentiate between novice and expert performance.
  • To evaluate the time and effort required for novices to reach a mastery learning level.

Main Methods:

  • Messick's framework for validity was applied.
  • Twenty novices and 9 experts completed 10 obstetric modules on a VR transabdominal ultrasound simulator.
  • Automated simulator metrics measured performance, with novices practicing to mastery.

Main Results:

  • One-third of simulator metrics distinguished between novices and experts (novice median 17.5% vs. expert median 90.0%).
  • High internal consistency (Cronbach's alpha=0.98) and acceptable test-retest reliability (ICC=0.62) were observed.
  • Novices achieved mastery learning in a median of 4 attempts (approx. 252 minutes of training).

Conclusions:

  • Validity evidence supports the use of VR ultrasound simulators for mastery learning assessment.
  • Ultrasound novices can achieve mastery learning in simulation-based training within 5 hours.
  • A significant portion of standard simulator metrics did not differentiate competence levels effectively.
