    Background:

    • Pneumolabyrinth is a rare inner ear condition characterized by air or gas within the labyrinthine structures.
    • This case explores a non-traumatic instance of pneumolabyrinth, initially suspected to be linked to labyrinthitis.

    Observation:

    • A 65-year-old male presented with acute vertigo and hearing loss following a cold.
    • CT imaging revealed air bubbles in the vestibule, semicircular canals, and cochlea.
    • Despite suspected bacterial infection and antibiotic treatment, no bacteria were cultured from otorrhea or a cochlear mass.

    Findings:

    • Exploratory tympanotomy did not reveal an obvious perilymphatic fistula.
    • A white mass from the cochlea contained white blood cells but no bacteria.
    • The patient was diagnosed with pneumolabyrinth secondary to labyrinth infection.

    Implications:

    • This case highlights the diagnostic challenges in non-traumatic pneumolabyrinth.
    • The etiology of air bubble formation in this context remains unclear.
    • Further research is necessary to elucidate the pathogenesis of pneumolabyrinth following inner ear infections.
