Summary

Trimethylaminuria, a metabolic disorder causing a fishy body odor, was diagnosed in a young boy through genetic testing. Dietary changes significantly improved his quality of life.

  • Trimethylaminuria (TMAU) is a rare inherited metabolic disorder.
  • It results from the inability to metabolize dietary trimethylamine (TMA).
  • TMA accumulation leads to a characteristic fishy body odor.

  • A 9-year-old boy presented with a distinct fishy odor affecting his hands, feet, saliva, and urine.
  • This unusual presentation prompted referral to secondary care for further investigation.

Findings:

  • Laboratory investigations, including urinalysis and genetic testing, confirmed the diagnosis of trimethylaminuria.
  • Genetic analysis identified the specific mutation responsible for the metabolic defect.
