Summary

Discharge planning processes are not fully normalized in practice, indicating a need for better integration. Evaluating existing practices is crucial before implementing new healthcare tools for sustainable improvement.

Background:

Discharge planning is vital for patient care transitions but often suffers from information exchange deficits.
Despite numerous initiatives over 30 years, failures in discharge planning persist, leading to medical errors and adverse events.
Theoretical frameworks like Normalization Process Theory (NPT) can aid in evaluating complex healthcare processes.

Purpose of the Study:

To explore the embedding and integration of the Discharge Planning Process (DPP) from the perspective of healthcare professionals.
To understand how DPP has been incorporated into daily practice among registered nurses, district nurses, and homecare organizers.

Main Methods:

An explorative study design utilizing NPT as a theoretical framework.
Data collection involved workshops, analysis of adverse events and system failures, web-based surveys, and individual interviews.

Main Results:

After 10 years, staff agreed on the definition and evaluation of discharge planning (coherence and reflexive monitoring).
Consensus was not reached on role allocation (cognitive participation) or execution (collective action) within the process.
These findings suggest the discharge planning process has not been fully normalized into daily practice.

Conclusions:

There is a need to analyze the implementation of existing practices to inform the development of new ones.
Understanding current practice is essential for creating effective and sustainable supportive tools in healthcare.
NPT provides a robust framework for analyzing and guiding the implementation of healthcare practices and interventions.
