Purpose of the Study:

  • To analyze the long-term functional status and prognostic factors following arterial repair of civilian upper limb injuries.

Main Methods:

  • Retrospective analysis of prospectively collected data from 108 patients undergoing 117 arterial repairs (1989-2010).
  • Long-term outcomes assessed via color Doppler ultrasound (patency), re-intervention rates, limb salvage, and the Disabilities of Arm, Shoulder, and Hand (DASH) questionnaire for functional status.

Main Results:

  • High long-term patency rate of 97% was observed after a median follow-up of 5.3 years.
  • Functional impairment was prevalent, with significantly higher DASH scores in patients with neurological injury (40.3 vs. 0.8) and ischemia at injury (4.2 vs. 0.0).
  • Blunt trauma was the most common cause (82%), with frequent associated nerve (36%) and orthopedic (60%) injuries.

Conclusions:

  • Arterial repair for upper extremity trauma yields favorable long-term patency rates.
  • Associated neurological injury and ischemia at the time of injury are significant determinants of long-term functional impairment.
  • While vascular repair is successful, addressing associated injuries is critical for optimizing functional recovery.
