Background:

  • Fatigue is a common and persistent issue in elderly individuals following myocardial infarction (MI).
  • Previous research suggests various factors like cardiac function, comorbidities, sleepiness, depression, anemia, inflammatory markers (interleukins), and social support may correlate with fatigue.
  • However, a systematic examination of these factors 6-8 months post-MI in an aging population is lacking.

Purpose of the Study:

  • To comprehensively investigate potential independent factors contributing to the development and persistence of fatigue in older adults 6-8 months after myocardial infarction (MI).
  • To identify specific clinical and psychological markers associated with post-MI fatigue in the elderly.

Main Methods:

  • A cohort of 98 participants (49 women and 49 men), aged 65-91 years, who were 6-8 months post-MI were enrolled.
  • Data collected included demographic and health status (heart rate, blood pressure, BMI, medications), comorbidity scales, fatigue assessments (Piper fatigue scale), sleepiness (Epworth sleepiness scale), depression (Geriatric Depression Scale), social support, and blood tests (B-natriuretic peptide, hemoglobin, interleukin-6).

Main Results:

  • Fatigue persisted in 76% of older adults post-MI, with no significant difference observed between sexes.
  • After controlling for age, interleukin-6 levels, and body mass index, only depression scores and mean arterial pressure emerged as statistically significant, linearly independent predictors of fatigue.
  • Specifically, higher depression scores (P trend = 0.0004) and elevated mean arterial pressure (P trend = 0.015) were associated with increased fatigue.

Conclusions:

  • Post-myocardial infarction depression and elevated mean arterial blood pressure are critical factors to assess when evaluating persistent fatigue in elderly patients.
  • These findings highlight the importance of addressing mental health and cardiovascular parameters in managing fatigue post-MI in older adults.
